Get a Library Card

How to get your very own Madison County Library card:

1. Print and fill out this form. Library Card Application. You can also get a form at the library. You’ll need a local address and telephone number, an ID with your address on it, and if you’re under 15, a parent or guardian to sign.

2. Bring your form to the Library and turn it in at the front desk to get a card.

3. You’re ready! Use your card to check out books, movies, use library computers, and online resources.
